rotation matrix

I am whipping together a model of a geometry problem and need the formulas
or algorithm for calculating a rotation matrix (3D) needed to align one
direction vector with another.  I can work it by hand if I have to but I
hate doing that when in a rush not wanting to search the inevitabl bugs.

Paper, book or code references welcome!
